Letters and Documentation – Jessica C. Lief

A significant part of my work with clients includes preparing documentation on professional letterhead that clearly identifies my credentials, outlines the client’s functional limitations, and supports specific accommodations when needed. These letters typically focus on how a condition affects major life activities and daily functioning.

Letters include my name, licensure, and contact information, along with relevant details about the therapeutic relationship, such as the length of treatment and frequency of visits. When clinically appropriate, I document the specific limitations that may affect a client’s ability to perform job duties, meet academic demands, or maintain appropriate housing.

I may also make specific recommendations, including modified schedules, specialized equipment, or the need for a service animal or emotional support animal.

I share only the minimum information necessary to support the requested accommodation. You will always have the opportunity to review letters and documentation before anything is sent.

I provide a wide variety of documentation and letters for clients. These have included:

  • Disability paperwork and letters
  • Accommodation paperwork for employment and school settings
  • Emotional support animal letters
  • Documentation supporting safe housing needs
  • Letters documenting the need for accommodations in testing environments
